IMPORTANT INFORMATION REGARDING YOUR MEDICAL BENEFITS UNDER THE LOCAL 464A WELFARE SERVICE BENEFIT FUND
Part-time Members who are enrolled in the Welfare Fund may enroll their eligible child dependents. “Eligible child dependents” include natural or adopted children of a Member up to age 26. Children involved in a legal adoption process will be covered when placed, provided the children otherwise meet the requirements of the Welfare Fund as a dependent.
Current part-time members who wish to have coverage for their eligible child dependents during the 2021 calendar year must enroll them during the annual enrollment period, which will be November 1 through November 30, 2020. If you do not enroll your eligible child dependents during this time, you will not have a chance to enroll them for coverage in 2020 unless you have a special enrollment event.
New part-time members who wish to have coverage for their eligible child dependents must enroll them within 30 days of their becoming eligible for coverage, and must pay the applicable premium established by the Trustees for dependent child coverage.
No child dependent may receive duplicate or additional benefits beyond the limits of the Welfare Fund by reason of both parents being qualified Members. The same parent must consistently claim any child dependent for coverage. No one may be covered as both a dependent and a Member. Where the child dependent has other coverage available, that coverage or source of payment must be pursued first. This Plan is not a Primary Plan or a Secondary Plan, but rather a Tertiary Plan and payer of last resort.
